Comprehensive consideration of the biological and social diversities of sex and gender as well as their interdependencies is mostly missing in human biomonitoring (HBM) studies. Using the INGER sex/gender concept as theoretical background, we analyzed differences in exposure to lysmeral, a compound commonly found as a fragrance in cosmetics, personal care, and household products, in 2294 children and adolescents in Germany using decision tree, regression, and mediation analysis. The variables “sex assigned at birth” and “age”, as well as well as use of personal care products and fabric conditioner proved to have the highest explanatory value. Mediating effects of behaviour associated with societal gender expectations were observed, as the use of cosmetics correlated highly with lysmeral metabolites concentrations in girls between 6 and 17 years, with the strongest effect in adolescents between 14 and 17 years old. In the youngest age group (3–5 years) boys showed higher concentration of the metabolite tert-butylbenzoic acid (TBBA) compared to girls of the same age but only if TBBA urine concentrations were normalized on creatinine. Our study offers the first retrospective sex/gender assessment of HBM data. It demonstrates the possibilities to rethink and broaden sex/gender analysis in existing HBM-studies and highlights the need for inclusion of new sex/gender concepts in the design of new studies.
Gender refers to sociocultural norms, identities, and relations that: 1) structure societies and organizations; and 2) shape behaviors, products, technologies, environments, and knowledge (Schiebinger, 1999). Gender attitudes and behaviors are complex and change across time and place. Importantly, gender is multidimensional (Hyde et al., 2018; Dubois et al., 2025) and intersects with other social categories, such as sex, age, socioeconomic status, sexual orientation and ethnicity (see intersectionality. Gender is distinct from sex (Fausto-Sterling, 2012). Historically, gender definitions have been based on a binary concept – women and men. In most key UN definitions, this binary framework continues to dominate, including in chemical policy. To avoid exclusions, newer definitions and guidelines often additionally use the term “all genders”. Beyond defining gender, applying a binary concept within the difference approach to Gender Mainstreaming remains valuable, productive and necessary, as it allows for comparing the experiences of women and men. Analyzing these differences and using binary-disaggregated data provides insights into social structures and inequalities. While we often work with binary categories for analytical purposes (also in chemicals management), we want to do so with a clear awareness of their limitations and with the aim of building more inclusive approaches in the future.
Sex refers to biology; however, there is no single, universally agreed-upon set of guidelines for defining sex. Sex is complex, dynamic, context-dependent, and interacts with gender and other social factors (Ritz & Greaves, 2022; Dubois et al., 2025).
In humans (and non-human mammals), sex traditionally refers to the biological attributes that distinguish male, female, and/or intersex according to functions that derive from the chromosomal complement (generally XX/XY), reproductive organs (internal and external genitalia), or specific hormonal, environmental, or cultural factors that affect the expression of phenotypic traits (Barresi & Gilbert, 2023; Jones, 2018; Kessler, 1998).
